In the Menu there are 3 branches with percentage numbers at the top. “You Only Live Once” is the middle one. It indicates how positive your relationship with Johnny Silverhand is. You get 10% of “You Only Live Once” Rating each for completing Rogue's sidequests "Chippin’ In" and "Blistering Love". When you visit Johnny’s grave during Chippin’ In, choose these dialog options to get another 5% rating: “The Guy who Saved My Life,” “Nah, fucked that up too,” “What do you want from me?” “Ok. As far as second chances go, this is your last,” “You were a real dick in the beginning,” “When you said you let down your friends…,” “Smasher biz really got to her,” “Yeah. I’ll call Rogue." You get another 10% each for completing the first two Kerry sidequests"Holdin’ On" and "A Like Supreme" that unlock after finishing Rogue's questline. This should add 45% to your rating and hopefully make you able to get the ending.
